This form signifies that your organization meets the designated criteria and that your organization will promote the activities to help address sports trauma and overuse prevention.
There are many other ways that medical institutes can be involved in the campaign, but the outlined participation agreement provides a basic foundation upon which medical institutes can promote athletic safety as part of their activities. Andrews Institute for Orthopaedics & Sports Medicine

In 2007, Baptist Health Care in Pensacola opened a world class institute for orthopaedics and sports medicine in Northwest Florida, with the leadership and reputation of nationally and internationally renowned orthopedic surgeon, James Andrews, MD. Children's Hospital of Wisconsin

Children's Hospital of Wisconsin Sports Medicine Program offers the only full-service sports medicine program in the state that is just for kids and young adults. The program also offers the state's only Concussion Clinic for kids and young adults, up to age 22. Our team consists of a combination of pediatric orthopaedic surgeons, primary care sports medicine physicians, nurse practitioners and physician assistants. Our goal is to make sure your kids are healthy – and can get back to the activities they enjoy – as soon as possible, but as safely as possible, too. We work closely with parents, coaches and other specialists to provide care plans that fit each child's specific needs. The Harkness Center for Dance Injuries at the NYU Hospital for Joint Diseases offers many subsidized and free services for the dance community including clinics staffed by orthopedists and dance physical therapists; state-of-the art research and rehabilitation technology; and free injury prevention screenings and lectures.
The Harkness Center was founded as a program of the Hospital for Joint Diseases in 1989 in response to the New York dance community's critical need for specialized and affordable health care. More than 50% of dancers are uninsured and cannot afford the medical help they need. NYU Langone Medical Center Hospital for Joint Diseases

The Hospital for Joint Diseases has a distinguished tradition of medical innovation. Since 1905 our physicians have pioneered a range of treatments that have improved the lives of people with musculoskeletal and neurological disorders. NYU Langone Medical Center and the Hospital for Joint Diseases (HJD) share a rich history, spanning HJD's academic affiliation with the NYU School of Medicine in 1986, its clinical association with NYU Langone Medical Center in 1994, the fusion of the two orthopaedic departments in 1997, and the integration of the rheumatology programs in 2001. Nathan Littauer Hospital

Nathan Littauer Hospital and Family of Health Services serves Fulton, Montgomery and Hamilton counties in upstate New York with a full-service 74-bed hospital, eight primary care centers, an 84-bed nursing home and a community education center. Penn State Hershey Bone and Joint Institute

The Penn State Hershey Bone and Joint Institute is affiliated with the Penn State Milton S. Hershey Medical Center and Penn State's College of Medicine.

- -Hershey - The Bone and Joint Institute is a 52,000 sq. ft. facility situated on the east campus of the Penn State Milton S. Hershey Medical Center, allowing for collaboration between specialists in orthopaedics, sports medicine, spinal disorders, hand surgery, metabolic bone disease and osteoporosis, rheumatology, radiology and chronic pain management. The site includes state of the art diagnostic x-ray, CT, and MRI imaging, as well as comprehensive therapy services and an outpatient rehabilitation facility which contains a unique underwater treadmill system to accelerate recovery.
- -State College - An integral component of the Bone and Joint Institute is our Penn State Orthopaedics practice site in State College, Pennsylvania, where we care for all Penn State University athletes as well as patients throughout the region.
The institute also includes researchers from both Hershey and State College, studying a wide variety of musculoskeletal disorders including arthritis, osteoporosis, biomechanics of implants, mechanisms of fracture healing, and cancer of the musculoskeletal system.
Physicians in the Institute work together to provide Sports Medicine care to the Hershey Bears Hockey Team, Harrisburg Senators Baseball Team, Penn State varsity athletes and local high schools.
Sutter Orthopaedic Institute

American Sports Medicine Institute is a 501(c)(3) not-for-profit sports medicine research and education foundation located in Birmingham, Alabama. ASMI was founded in November 1986 by orthopaedic surgeons Dr. James R. Andrew and Dr. Lawrence J. Lemak.

The purposes of the Institute are to:

New York Hospital Queens Department of Orthopaedics and Rehabilitation

The Orthopaedics & Rehabilitation Department at New York Hospital Queens (NYHQ) offers the community the most sophisticated, high-quality orthopaedic care available anywhere today – this is why we are now known as The Bone and Joint Hospital of Queens.

NYHQ is the only hospital in Long Island and Queens, and one of only three sites nationwide, to offer patients the HemiCAP(R) Focal Resurfacing system, an investigational procedure designed to repair injured knee cartilage while protecting healthy cartilage to help prevent further damage. Missouri Orthopaedic Institute

The Missouri Orthopaedic Institute is central Missouri's largest freestanding orthopaedic center, as well as the region's most comprehensive orthopaedic surgery center. Luxembourg Olympic Medical Center

The Luxembourg Olympic Medical Center is located at the Centre Hospitalier – Clinique d'Eich in Luxembourg, a 525 bed public hospital serving as a regional referral centre. It has been created in 2008 following an initiative of the Luxembourgish Olympic Committee with the mission to offer high-quality services to its athletes and to work on sports injury prevention initiatives.
