This form signifies that your organization meets the designated criteria and that your organization will promote the activities to help address sports trauma and overuse prevention.
There are many other ways that medical institutes can be involved in the campaign, but the outlined participation agreement provides a basic foundation upon which medical institutes can promote athletic safety as part of their activities. Review the starter list of involvement activities and ideas and then develop additional ways to promote injury prevention as a part of youth sports.

Andrews Institute of Sports Medicine

In 2007, Baptist Health Care in Pensacola opened a world class institute for orthopaedics and sports medicine in Northwest Florida, with the leadership and reputation of nationally and internationally renowned orthopedic surgeon, James Andrews, MD. The creation of the Andrews Institute has secured Northwest Florida as a cradle for musculoskeletal treatments and research. Children's Hospital of Wisconsin

Children's Hospital of Wisconsin Sports Medicine Program offers the only full-service sports medicine program in the state that is just for kids and young adults. The program also offers the state's only Concussion Clinic for kids and young adults, up to age 22. Our team consists of a combination of pediatric orthopaedic surgeons, primary care sports medicine physicians, nurse practitioners and physician assistants. Our goal is to make sure your kids are healthy – and can get back to the activities they enjoy – as soon as possible, but as safely as possible, too. We work closely with parents, coaches and other specialists to provide care plans that fit each child's specific needs. Our specialists partner with physical and occupational therapists to provide a comprehensive range of services for kids including rehabilitation of injuries, bracing, guidelines for safe return to play, injury prevention techniques and education.
